Y'see, Kim and I have worked together multiple times by now... and we've not only always used a safeword, but it's always been the same safeword. So, this time I didn't bother to remind her or double-check with her that she knew that! As a result, Kim not only forgot her safeword, but assumed that because I didn't mention it, we weren't going to use one!
